Wave 2
Data Modernization Implementation Center Program
The Public Health Data Modernization Implementation Center (IC) Program will be launching Wave 2 in the Fall of 2025. Wave 2 will focus on advancing electronic case reporting (eCR) capabilities across jurisdictions. Wave 2 will support up to 60 Public Health Agencies (PHAs) between early 2026 and the end of the program in the Fall of 2027. Wave 2 is the final wave of the IC Program.
Applications
The application process for Wave 2 is now open. The application will close at 11:59 p.m. ET on October 17, 2025, and no late submissions will be considered for selection. Selection of Wave 2 participants is anticipated to be completed by the end of 2025. Only one application may be submitted per PHA and cannot be amended after submission; however, applicants may save their application and continue to work on it until the deadline on October 17, 2025.
Selected projects will be categorized into three strategic tracks, informed by CDC’s priority areas and IC Program goals:
- Processing electronic initial case reports (eICRs) into surveillance systems
- Using intermediaries and modern approaches to optimize eCR data exchange and usability
- Intermediary demonstrations of eCR data exchange via Health Data Utilities (HDUs)
Eligibility
To receive implementation project support, all PHAs, regardless of strategic track, must:
- Have a complete, up-to-date (i.e., not expired) SAM.gov registration.
- Submit a Letter of Support from their State/Territorial Health Official (if a state/territory/freely associated state) or a Letter of Attestation indicating that they have notified their state-level DMI director about their application (if a local).
- For non-ELC/A3-funded PHAs, projects should demonstrate state involvement or support (i.e., focusing on improving local access to eCR data through their state or optimize their use of eCR data received through their state).
- Proposed implementation projects must be feasible within program timelines and provided levels of support.
- PHAs currently supported in Wave 1 of the IC Program are eligible to apply for and receive support through Wave 2. As this is a competitive process, no preference will be given to those PHAs currently in Wave 1.
All Wave 2 projects must be completed by September 2027, coinciding with the end of the IC Program.

The IC Program will be hosting two Q&A Sessions where PHAs can attend and ask questions related to Wave 2 applications, selection, timelines, and appropriate scope. Links to those sessions can be found below when available. Attendance is optional, but encouraged and open to all PHAs considering applying for Wave 2.
